Download Instructions

In order to view and print the downloaded documents, you will need the Adobe Acrobat Reader (version 5.0 or higher) and access to a printer. Acrobat Reader users have the ability to complete the form directly within Acrobat Reader prior to printing it. Please note, Adobe Acrobat Reader cannot save the data you entered to fill in the application.

If you don't have the Acrobat Reader, you can obtain it free of charge via the Adobe web site (installation instructions are also provided): http://www.adobe.com/products/acrobat/readstep2.html

Some helpful tips for downloading our documents are provided below. Once you have a copy on your local hard drive, you can view the application booklet and print out several or all pages by using Acrobat Reader.

*Note that the Acrobat Reader v7 will take up about 20 MB of disk space when installed on your computer.

Some helpful downloading tips:

  1. Note that our forms are in the PDF file format, so you will need the proper version of the Acrobat Reader installed on your computer. Do not try downloading the application until the proper version has been installed.

  2. When you click on the download link for either version of the application, one of two events should happen:

    1. If you do not have the Acrobat Reader plug-in installed: A window should appear with an option to save it to a directory on your hard drive. After saving the file to your computer, you can open the PDF file using the Adobe Acrobat Reader. Double click on the Acrobat Reader icon to run the program. Once it has finished loading, select "Open" from the File menu.

    2. If you have the Acrobat Reader plug-in installed: After the download completes, the application should be displayed in your browser window. While you can fill in the application within your browser, due to it's length, you would likely find it better to save a copy onto your hard drive. To save a copy of the PDF onto your computer, click on the small floppy disk icon on the toolbar just above the displayed application, and save it to a directory on your hard drive. To open the saved application, double click on the Acrobat Reader icon to run the program. Once it has finished loading, select "Open" from the File menu.

*Note: Some computers may take a while to download and open the application PDF.

  3. You should now be able to view and print the document (you may want to print only the pages you have to mail). If you are still having problems with the Adobe reader, click here for Adobe Customer Support.

Important Notes On Filling in the Application Prior to Printing

As the application file is fairly large, we recommend that you save the file and close your internet browser. Even though the plug-in appears within the browser, it may be ideal to conserve memory while filling out the application.

Please note: While the Acrobat Reader does have a "Save" option, it will NOT save any information you may have entered into the forms. With this in mind, you may want to either complete the application all at once or print out each of the completed pages at your convenience.

More On Filling Out PDF Forms (Windows and Macintosh)

To fill out a form:

  1. Select the hand tool.

  2. Move the mouse cursor over a form field until it changes into an 'I'-beam, and click. You will then see a flashing cursor in the form field where what you type will appear. Additionally, the mouse cursor will change into a hand that is pointing when you are over a checkbox. If you click, you will check or uncheck the checkbox.

  3. After entering text or checking a checkbox, choose from the following:
    • Press Tab to accept the field change and go to the next field.
    • Press Shift+Tab to accept the field change and go to the previous field.
    • Press Return or Enter to accept the field change and deselect the current field.

Note: In a multi-line text field such as 'Academic Honors,' return goes to the next line; it does not accept the field change or deselect the current field.

    • Press the 'Escape' key to reject the field change and deselect the current field. If you press the Escape key a second time and you are in Full Screen mode, you will exit out of Full Screen mode.

  4. After you have completed, or partly completed, the application you can print the application as you complete each page.

To clear a form:

Exit the Acrobat viewer and start again. Note: This will clear the entire form! You should print the pages that have been completed before doing this.

Note: Clicking the 'Reload' button or the 'Go Back' button, or following a link while using your internet browser does not clear the form.
